Tim Russert's heart scan score 210. . .in 1998

Despite the media blathering over how Mr. Russert's tragic death from heart attack could not have been predicted, it turns out that he had undergone a heart scan several years ago.

A New York Times article, A Search for Answers in Russert’s Death, reported:

Given the great strides that have been made in preventing and treating heart disease, what explains Tim Russert’s sudden death last week at 58 from a heart attack?

The answer, at least in part, is that although doctors knew that Mr. Russert, the longtime moderator of “Meet the Press” on NBC, had coronary artery disease and were treating him for it, they did not realize how severe the disease was because he did not have chest pain or other telltale symptoms that would have justified the kind of invasive tests needed to make a definitive diagnosis. In that sense, his case was sadly typical: more than 50 percent of all men who die of coronary heart disease have no previous symptoms, the American Heart Association says.

It is not clear whether Mr. Russert’s death could have been prevented. He was doing nearly all he could to lower his risk. He took blood pressure pills and a statin drug to control his cholesterol, he worked out every day on an exercise bike, and he was trying to lose weight, his doctors said on Monday. And still it was not enough.

“What is surprising,” Dr. Newman said, “is that the severity of the anatomical findings would not be predicted from his clinical situation, the absence of symptoms and his performing at a very high level of exercise.”


Buried deeper in this article, the fact that Mr. Russert had a heart scan score of 210 in 1998 is revealed.

That bit of information is damning. Readers of The Heart Scan Blog know that heart scan scores are expected to grow at a rate of 30% per year. This would put Mr. Russert's heart scan score at 2895 in 2008. But the two doctors providing care for Mr. Russert were advising the conventional treatments: prescribing cholesterol drugs, blood pressure medication, managing blood sugar, and doing periodic stress tests.

Conventional efforts usually slow the progression of heart scan scores to 14-24% per year. Let's assume the rate of increase was only 14% per year. That would put Mr. Russert's 2008 score at 779.

A simple calculation from known information in 1998 clearly, obviously, and inarguably predicted his death. Recall that heart scan scores of 1000 or greater are associated with annual--ANNUAL--risk for heart attack and death of 20-25% if no preventive action is taken. The meager prevention efforts taken by Mr. Russert's doctors did indeed reduce risk modestly, but it did not eliminate risk.

We know that growing plaque is active plaque. Active plaque means rupture-prone plaque. Rupture prone plaque means continuing risk for heart attack and death. Heart attack and death means the approach used in Mr. Russert was a miserable failure.

While the press blathers on about how heart disease is a tragedy, as Mr. Russert's doctors squirm under the fear of criticism, the answers have been right here all alone. It sometimes takes a reminder like Mr. Russert's tragic passing to remind us that tracking plaque is a enormously useful, potentially lifesaving approach to coronary heart disease.

Who needs to go next? Matt Lauer, Oprah, Jay Leno, some other media personality? Someone close to you? Can this all happen right beneath the nose of your doctor, even your cardiologist?

I don't need to remind readers of The Heart Scan Blog that heart disease is 1) measurable, 2) trackable, 3) predictable. Mr. Russert's future was clear as long ago as 1998. Every year that passed, his future became clearer and clearer, yet his doctors fumbled miserably.

    I wonder if he was taking fish oil supplements to try to drive down his triglycerides and niacin to prop up his HDL?

    I had a 234 score in 2005 and a 419 score in 2007 - if it wasn't for resources like TYP - I wouldn't have pushed my Dr with questions about Vit D and CQ 10 and Fish Oil...  sit waiting for the next scan to see if things are under control (now - small LDL-P 123 nmol/L).

    Just think if Tim R had the time to do a bit of research himself and found TYP - but that is what your physicans should be doing for you.... growing... learning... but as an engineer, I know the spectrum of people calling themselves engineers is a large spectrum... so it is with MDs.

    Yes. Fish oil alone could have cut his risk of sudden cardiac death by 45%. It would have cost him all of $3 per month.

    Dr Davis I just wonder what you think of this Dr. Mehmet(?) Oz who keeps popoing up on television and writing books talking about the same old stuff, low fat, high carbs blah blah blah . . . I think since Mr. Russerts death I've seen him on tv 3 times and NOT ONCE has he mentioned calcium scoring, vitamin D, fish oil . . .

    What a tragedy.  All week long I have been asking myself how such a smart man could be so uninformed about his own health?

    With all the resources at Mr. Russert's disposal, I would think he could have easily learned more about his condition, and the measures he might have taken to save himself.  [Then too, he might have also come across the Track Your Plaque website... or the book.]  Instead, he was apparently greatly trusting of his internist and cardiologist, and perhaps thought he was receiving optimal medical management... and nothing more could be done?

    Beyond that, I wonder about his Vitamin D status, and whether he was dehydrated from the long flight back from Europe?  I also wonder if the emotional stresses (good and bad) of a quick trip to Europe, his son's graduation from college, and having recently placed his beloved father into a care home, on top of what could only be termed a stressful and grueling work life (no matter how much he may have loved it) might have lead his body to the tipping point on that day.   I suppose we are unlikely to have these answers under the circumstances.

    R.I.P. Mr. Russert, but shame, shame, shame on your physicians, IMO they really let you down.

    Thanks for this truthful blog, an antidote to all the media nonsense and C.Y.A. I have seen in the past few days.

    My wife has been to Dr. Michael Newman the internist for Tim Russert.  I don't think she is going back.  I had Angioplasty about 10 years ago with stents put in my right and left artery.  Since then I have the thallium stress test every year, take 1500 mg's of niaspan a day, Lipitor, a blood pressure lowering drug, and aspirin.  Both my cardiologist, and my internist state that a heart scan would not do me any good, and my cardiologist stated that the heart scan would simply confuse the issues.  Are they right? Would the heart scan harm me?  If so, how?

    In response to the comment by sschein, I'm not sure it's such a great idea to have a thallium stress test every year.  You should probably investigate the possibility of a CT-angiogram.  

    I am not a doctor so I don't want you to think I'm defending them, but there's only so much that a doctor can do in the office visit environment.  It's really up to the patient to do the research and decide on what he believes is the best course of treatment for him or herself and then try to bring the doctor around to his point.  In my own case I refuse to have a thallium stress test and have finally decided to have a 320 slice CT-angiogram when I go to Boston next month.  My cardiologist may not agree that it's the choice he'd choose, but he's going along with it.  Quite simply they don't have the time to convince the patient one way or the other.  We really don't know all the details about Tim Russert's care.  If he had his own private physician who tended only to him or who was consulted extensively I'd probably expect better.  As just one patient (admittedly a famous one) I'm not sure how much you can expect from a doctor.  If he suggests a stress test or an angiogram and you think better of the idea, it's up to the patient to chart his own course.

Charles was visibly confused.

He'd gotten his CT heart scan after hearing one of the local scan center's ads on the radio. His score 2773, obviously in the 99th percentile for any age.

"Do you think the score means anything? My primary doctor said that it was meaningless because it was all in the deep wall of the artery. He said that it has nothing to do with risk for heart attack. As long as I feel good, he says don't do anything."

What exactly did his doctor mean, in the "deep wall of the artery"?

What the doctor is referring to is the fact that some people with a long history (many years) of diabetes or kidney failure (also for many years) tend to develop calcium deposits in the media, or muscular layer of arteries. The media is the tissue thin layer just below the intima, the most inner layer of arteries that we usually associate with atherosclerotic plaque and the layer that is most prone to calcium accumulation that we score on heart scans.

Aging, generally into your late 70s, 80s, and onwards, also increases the likelihood of medial calcification. Lastly, longstanding deficiency of vitamin D encourages medial calcification.

Is there any way to distinguish intimal vs medial calcification on a heart scan? No, there is not. Having read many thousands of CT heart scans, I can tell you that there is no practical way in 2007 to tell the difference.

Then how did this doctor "know" that Charles' calcium was "deep walled" or medial? Simple: He didn't. This was yet another example of ignorance based on old thinking. Unfortunately, he did Charles a serious disservice by dismissing his heart scan score that predicted a 25% per year risk for heart attack.

Interestingly, whether calcium is intimal as in atherosclerotic plaque, or medial, both are strongly associated with risk for heart attack. In other words, if calcium is confined to the intima, heart disease risk is present. If calcium is limited to the media, risk is still present.

In all practicality, the only difference we make of the intima vs. media argument (that is, when the distinction has been made by some other means like intracoronary ultrasound, the test that is truly necessary to distinguish the two patterns) is that medial calcification may be more powerfully related to vitamin D deficiency. Thus, someone with heavy medial calcification may require closer attention to maintaining a perfect year-round blood level of 25-OH-vitamin D3. But that's the only practical difference.

    I am 72.
    I recently had a CTA scan with "no detectable paque"
    I am also aware of recent research which shows evidence of menaquinone both preventing and reversing calcification.
    Is scanning thought to be less sensitive to medial calcification (as opposed to intimal), and at risk of being 'missed'?

    If so would preventive menaquinone be justified in a 'clean' case like mine?

    Hi, Mike-
    No, the scan quite reliably detects both intimal and medial calcification. Taking K2 is very optional. How about some traditional, fermented cheese? I do not believe that K2 supplementation would yield substantial heart benefits. However, if bone health is in question, that migyht be a reason.

This is your brain on wheat II

In the original Heart Scan Blog post, This is your brain on wheat, I discussed how opioid peptides (i.e., small proteins that act like opiates such as heroine or morphine) that result from digestion of wheat cause unique effects on the human brain, particularly addictive behaviors. I also briefly reviewed how elimination of wheat has been shown to reduce auditory hallucinations and other psychotic behaviors in a subset of people with paranoid schizophrenia.

These two phenomena, addictions and schizophrenia, are most likely the result of exorphins that cross the blood-brain barrier. Exorphins--exogenous morphine-like compounds--can be blocked by opiate-blocking drugs like naloxone and naltrexone. Naloxone is used in hospitals to reverse morphine or heroine overdoses; naltrexone is being repackaged into a weight loss drug, since blocking wheat-derived exorphins reduces appetite. (Yes: The USDA tells us to eat more wheat, the drug industry sells us the antidote.)

There's another way that wheat can affect the brain and nervous system: immune-activated damage.

This is similar to the effect seen in celiac. There's even overlap with some of the antibody markers used to diagnose celiac, like the anti-gliadin antibodies and the anti-endomysium antibodies.

The most common immune neurological syndrome consequent to wheat consumption is cerebellar ataxia, a condition in which an immune response causes damage to the Purkinje cells of the cerebellum, the portion of the brain responsible for balance and coordination. This results in stumbling, incoordination, incontinence, and eventually leads to reliance on a cane or walker and wearing a diaper. Average age of onset: 53 years. A shrunken, atrophied cerebellum can be seen on an MRI of the brain.

Problem: Most people with central nervous system damage caused by wheat do not have any intestinal symptoms, like diarrhea and abdominal pain, the sort of symptoms usually associated with celiac disease. It means the first sign of wheat-induced brain damage may be bumping into walls and wetting your pants.

    It only makes sense that there are vast numbers of people actually addicted to food, not metaphorically, but in the same way people are addicted to drugs and nicotine. A good start would be stop subsidizing this addiction, but since we have a government of the grain farmers, by the grain farmers, and for the grain farmers, that's not likely.

    As usual you are SPOT ON. exactly right with the symptoms and age. Just amazing all clinical symptoms described were seen by me in my father from 53 (stumbling and falling) to 58 (requiring help walking) to 60 (epilepsy hallucinations and fears)to 61 (bedridden) to 64 (last year November) death.

    Come to think, it was so simple to save him. It is just unreal.

    Is it time to join the bandwagon? No more drinking, no more smoking, no more wheat? Really, did I just write that? I have to say, I really enjoy my PB&J on whole grain wheat bread, as do my kids.

    Eating wheat is like apple pie and July 4th fireworks. How can we possibly do without and find a relatively cheap substitute? Is rice any good or is that a bad carb too?

    To eat healthy, is it just you need to eat organic and nonwheat foods and watch your carb-mix?

    Does it have to be this complicated? Has anyone written a book with easy to find, cheap/healthy ingredients that is easy to prepare in 30 minutes or less and feeds a family of 4?

    Right now, we're basically down to grilled chicken/fish/pork with steamed fresh brocolli/green beans and long-grain rice. That's pretty much all we eat anymore, with the occassional cheeseburger/steak indulgence.

    I started Low Dose Naltrexone 2 months ago to help with Autoimmune Disease.  I started at 1.5 and now am at 3.0
    I will increase to 4.5 in 2 weeks.

    I eliminated grains and dairy 1 month ago.

    I have lost 10 pounds.

    I could be as simple as the diet changes but I think more is going on.

    I have less pain which allows me to sleep through the night.
    I have more energy.
    I am more active and actually exercising.
    I am supplementing Vit. D and getting daily sun exposure (my Vit. D level was 41).
    My moods have greatly improved.

    Ironically, any time I have been prescribed an opiate pain medication, I have had severe allergic reactions.

    As far as the Neuro symptoms, I do have Meniere's complete with dizziness and vertigo.  So far I have not noticed any positive impact but still hopeful.

    I know you are right on. I was having mild ataxia and stress incontinence. Off gluten for 7 years and balance is better and no stress incontinence.

    This also affects dogs. My 12 year old cairn terrier was stumbling, falling over and urinating in her sleep. Got her off grains 2 years ago and she improved immediately.

    i'd agree with Trevor as well.

    sourdough wheat (traditional preparation) and boiled raw milk go together.

    sourdoughing helps breakdown anti nutrients in wheat making the nutrients more bio available. Further Raw milk takes care of the rest by providing necessary enzymes (phystase etc) to digest wheat completely.

    pasteurized milk and wheat consumed without sourdoughing give both milk and wheat a bad name and will improve health when stopped simultaneously.

    traditional preparations eliminate such problems to a large extent.

    Celiac disease is a digestive disease that damages the small intestine and interferes with absorption of nutrients from food.  Is an inherited, autoimmune disease in which the lining of the small intestine is damaged from eating gluten and other proteins found in wheat, barley, rye, and possibly oats.

    Sprouting wheat begins the process of breaking down gluten, but it is not a complete process. Same goes for fermenting. Making a suboptimal food less bad for you does not mean that food is now good for you.

    As for boiled raw milk, taking raw milk to a boil heats it to an even higher temperature than is done during regular, non-UHT pasteurization, and it keeps it at that high temperature for a much longer time than any commercial pasteurization process. Raw milk that's been pasteurized at home at a higher temperature for a much longer time is not somehow magically superior to commercially pasteurized milk.

    Just out of curiosity, I would like to know what is the point of buying something raw (supposedly because "raw" holds more benefits) only to then get it home and cook it. Boiling raw milk, in my estimation, defeats the purpose of consuming raw milk. Boiling kills everything. I buy raw milk weekly and I drink it "raw." That's why I buy it.  
    Am I missing something? (serious question).

    Anonymous, I don't have acute gluten sensitivity, but I've read enough about gluten sensitivity to know that sprouting and fermentation are not 100% effective at making wheat a tolerable food for people with gluten sensitivity.

    Why cling desperately to consumption of a crap quality food when it's so much easier and simpler to just not eat it at all? One personal experience I can draw on is the addictive nature of wheat. I've been addicted to both tobacco and alcohol, but the most addiction-triggering image I can visualize in my mind is a loaf of locally made, crusty Italian bread. I think people cling to wheat consumption because it's addictive, plus it's deeply embedded in human culture.

    raw milk is a relatively new fad in usa while india is the highest wheat and milk consumer since hundreds of years. The way they consume raw milk, is, after boiling it and the way they consume whole wheat is after making sourdough.

    I personally consume raw milk without boiling but whats important is to understand the effects of consuming wheat and milk traditionally on health viz a viz consuming it in modern style.

    Alex wheat is sub optimal as are many other foods. the only complete food is milk, everything else is had in combination with a complementary food.

    Wheat is also not easy to avoid while its consumed traditionally  daily in the east, it is everywhere in its modern avatar in the west.

    its not a bad idea to figure out wheats' complement and how it works than declare wheat suboptimal and write it off.

    I think that more than the problems wheat may cause for some, the problem is the amount of wheat we consume. Consuming the bulk of your calories from wheat (or grain) is a problem, even for those who don't have any existing conditions. Drinking beer all day or more than you should isn't good either but that doesn't mean that a beer here and there or even one per day is a big deal. For an alcoholic one beer is a bad thing but for the average person 1 or 2 isn't. For someone with a problem, wheat is bad; for the average person a little here and there in moderation isn't. There are a lot of things modern man eats that he didn't eat at one time. But then again, there are many things in life in general that modern man does that we didn't do years ago. We will always look to make things easier and in doing so compromise ourselves in some way. The best thing is to be educated enough to make good decisions but not get too carried away in either direction.
    Eating store bought chicken and meat tainted and chemically enhanced isn't good either. What does that do to us long term? What about our children. Eat less wheat and grains and avoid one illness but get another from mystery meat. So I guess we can't win no matter what we do. We can't get crazy, we just have to make good decisions.
    Middle of the road always seems like a good starting point.

French researcher, Dr. Michel de Lorgeril, has been in the forefront of thinking and research into nutritional issues, including the Mediterranean Diet, the French Paradox, and the role of fat intake in cardiovascular health. In a recent review entitled Recent findings on the health effects of omega-3 fatty acids and statins, and their interactions: do statins inhibit omega-3?, he explores the question of whether statin drugs are, in effect, incompatible with omega-3 fatty acids.

Dr. Lorgeril makes several arguments:

1) Earlier studies, such as GISSI-Prevenzione, demonstrated reduction in cardiovascular events with omega-3 fatty acid supplementation, consistent with the biological and physiological benefits observed in animals, experimental preparations, and epidemiologic observations in free-living populations.

2) More recent studies (and meta-analyses) examining the effects of omega-3 fatty acids have failed to demonstrate cardiovascular benefit showing, at most, non-significant trends towards benefit.

He points out that the more recent studies were conducted post-GISSI and after agencies like the American Heart Association's advised people to consume more fish, which prompted broad increases in omega-3 intake. The populations studied therefore had increased intake of omega-3 fatty acids at the start of the studies, verified by higher levels of omega-3 RBC levels in participants.

In addition, he raises the provocative idea that the benefits of omega-3 fatty acids appear to be confined to those not taking statin agents, as suggested, for instance, in the Alpha Omega Trial. He speculates that the potential for statins to ablate the benefits of omega-3s (and vice versa) might be based on several phenomena:

--Statins increase arachidonic acid content of cell membranes, a potentially inflammatory omega-6 fatty acid that competes with omega-3 fatty acids. (Insulin provocation and greater linoleic acid/omega-6 oils do likewise.)
--Statins induce impaired mitochondrial function, while omega-3s improve mitochondrial function. (Impaired mitochondrial function is evidenced, for instance, by reduced coenzyme Q10 levels, with partial relief from muscle weakness and discomfort by supplementing coenzyme Q10.)
--Statins commonly provoke muscle weakness and discomfort which can, in turn, lead to reduced levels of physical activity and increased resistance to insulin. (Thus the recently reported increases in diabetes with statin drug use.)

Are the physiologic effects of omega-3 fatty acids, present and necessary for health, at odds with the non-physiologic effects of statin drugs?

I fear we don't have sufficient data to come to firm conclusions yet, but my perception is that the case against statins is building. Yes, they have benefits in specific subsets of people (none in others), but the notion that everybody needs a statin drug is, I believe, not only dead wrong, but may have effects that are distinctly negative. And I believe that the arguments in favor of omega-3 fatty acid supplementation, EPA and DHA (and perhaps DPA), make better sense.

    This is a (too) far stretched conclusion pretty obviously. O3s still haven’t shown any consistent benefit alone, with a statin drug or without it, and cherry picking the studies that seem to look doesn’t help that fact either! The data is just inconclusive.

    A much more interesting blog article I hope to read in the future would be about niacin!
    Niacin has shown more than once to produce favourable outcomes alone and combined with resins. (CDP, FATS, etc.) On the other hand there can be no more doubt that it doesn’t add any benefit to statin therapy after the results of the two big clinical endpoint studies (AIM-HIGH and HPS2-THRIVE), that flush the cute little landmark studies (ARBITER, HATS, etc.) down the toilet where they belong.
    So what does that imply for the TYP-programm? A rhetorical question it should be, but I’m sure somebody will add some woo…

    It is interesting that he should be doing this research into how statins react with Omega-3's. Statins may be one of the most widely prescribed drugs at this point, and I certainly think more research should be conducted on their long-term effects. I recently read about another study which looked at the effects of exercise and statins. Apparently, this drug actually works to counteract some of the benefits that are gained by getting regular exercise for people with cholesterol problems.

    Stephen in Jacksonville.   There's no maybe to it.  The number 1 and number 2 selling drug in the world are statin drugs.  32 million Americans are currently taking statin medication.  The one thing that the 32 million have in common is:  THEY HATE TAKING THEIR MEDICATION.
